Sample submission form and sample ID information

When sending samples to Biosearch Technologies, it is essential to provide as much information as possible using our sample submission and plate map documents.

For all NAP projects, Biosearch Technologies requires the following documentation from the customer - sent prior to sample arriving on site:
  1. A completed sample submission form for extraction services - this must include details of the sample source and type, the sample volume (or weight for tissue), customer contact details, and information about any additional services that you may require
  2. Sample ID information – this can be submitted in one of two ways:
  • A completed plate map file – this must be used if samples are being submitted to Biosearch Technologies in plate format.  
  • A list of sample IDs in Excel (sample manifest) – this should be used if samples are being submitted to Biosearch Technologies in tube format. Individual tube IDs should not exceed 15 characters. (Please note: tubes should be sent in racks, with the samples placed in the same order as detailed in the manifest).
